We currently have an exciting opportunity for an Education, Health & Care Coordinator at our Rainbow Family Centre (which offers an inclusive nursery service for both mainstream children, and those with special and complex needs) to help us deliver a fun, safe and nurturing environment for children.

We deliver a range of exciting, creative and innovative specialist and inclusive services to children and young people with disabilities including those with complex physical and learning disabilities, communication disabilities such as autism and behaviours that can challenge.

We currently have an exciting opportunity for an Education, Health & Care Coordinator at our Rainbow Family Centre (which offers an inclusive nursery service for both mainstream children, and those with special and complex needs) to help us deliver a fun, safe and nurturing environment for children.

This new position will be a pivotal role in supporting children in accessing nursery spaces and progressing to school with the appropriate support in place to achieve their Early Years Foundation Stages.

  • You will be based at our newly refurbished Rainbows nursery based near Queens Park which provides year round places from 6 months to 5 years.
  • A key element of the job is to support the completion of ECHP plans which gather evidence of need to support access to funding.
  • The role includes liaison with health centres and other referrers, delivering portage in family homes, and supporting children’s transition into and through education.

The post holder will be an excellent communicator and skilled in report writing as well as someone willing and able to play with children with special educational needs and support family resilience through listening and signposting to additional support as well as supporting self-advocacy.

We offer continuing opportunities for learning and development together with regular supervisions and support, Subject to enhanced DBS checks and the take up of references and you must be over 18 years old to apply.
