Family Services Team

Adults

There are currently over 160 families known to the Family Services Team, who receive visits from a Family Advisor, on varying levels, depending on their individual needs.

Our friendly staff include:

  • Sharon Sage, Family Services Manager
  • Anna Bhalla, Family Advisor
  • Jane Munn, Family Worker
  • Fina D'Emmanuel, Family Worker

We hold regular information and consultation groups for families. We also attend the St Marys Learning Disability Working Group, to ensure the needs of parents and Carers are represented.

Children

There are currently ninety four families known to the Family Services Team, who receive home visits and attend Rainbow Family Centre for some visits.

We have ongoing involvement in the User Involvement Forum and with the teams that have relocated to Woodfield Road.

We work in partnership with the Childrens Services Manager from the Primary Care Trust, supporting parents with the ongoing consultation process in relation to the co location of services delivered from Woodfield Road.

We have a parents information group running once every six weeks with a range of speakers or presentations on issues requested by parents.

We attend the interagency Disability Strategy Group and support a parent representative to also attend the meeting. We also attend the transition Steering Group.

We have worked in partnership with the Parent Partnership Co-ordinator within the  Special Educational Needs department to provide information, training and advice to parents when appropriate.

Service

We support parents carers and people with disabilities in a variety of ways.

  • We visit people on a one to one basis in their own homes or here at Rainbow.
  • We offer emotional support.
  • We support people to self advocate.
  • We advocate on people's behalf when appropriate.
  • We provide a mediation role when relationships have been damaged with other Service Providers, and ensure vital services continue to be delivered.
  • We link people to both voluntary and statutory agencies when necessary and liaise with these agencies on behalf of families.
  • We provide information to people on available services and how to access them in a way that is straightforward uncomplicated and not jargonistic.
  • We offer opportunities to families to partake in consultation processes to shape the future of services they receive.
  • We provide social and leisure opportunities though our annual family day trip, parties at Christmas and during the year.
